Healthy Endeavors provides comprehensive ADHD evaluations via Telehealth across Arizona and in person at our Tempe office. Our goal is to help families understand what’s really happening and develop practical strategies that make daily life easier.
Transparent Pricing: Insurance accepted. We also offer a Flat Fee of $600 (Payable via credit card, HSA, or FSA).
What Your Evaluation Includes:
A 45-minute comprehensive intake with a licensed nurse practitioner.
A 30-minute follow-up session to review evaluation forms.
A 30-minute executive functioning behavior training session
An official diagnosis (if criteria are met).
A personalized treatment plan.
One school accommodation (504/IEP) letter if needed
Optional follow-up care for medication management, executive functioning coaching, and school accommodation (504/IEP) letters is available for $85 per 30-minute session.
How It Works: Our 4-Step Evaluation Process
Step 1: Book Your Consultation. Schedule a convenient telehealth or in-person intake appointment, typically available within 1-2 weeks.
Step 2: Comprehensive Evaluation. We will conduct a 45-minute clinical interview and review detailed behavioral forms. If it's needed, we'll obtain additional evaluations from parents and teachers.
Step 3: Executive Functioning and Behavioral Assessment: We will conduct a 30-minute session focused on one of the areas of executive functioning that needs strengthening.
Step 4: Clear Answers & A Plan. In our 30-minute follow-up, we will discuss the official diagnosis (if applicable) and build a personalized roadmap for your child's success.

What Happens During an ADHD Evaluation?
An ADHD evaluation helps identify whether symptoms of inattention, hyperactivity, and impulsivity are consistent with Attention-Deficit/Hyperactivity Disorder. Our assessments include:
A detailed medical and developmental history
ADHD screening questionnaires and rating scales
Review of school concerns and behavior patterns
Screening for related conditions such as anxiety or depression
A clinical assessment by an experienced nurse practitioner

Whether we are evaluating a 7-year-old struggling with classroom behavior, a teenager overwhelmed by high school, or a young adult having trouble managing college or career demands, our assessments are custom-tailored.
For Children & Teens: We heavily incorporate parent and teacher input, academic assessments, and developmental history to provide recommendations for school supports (IEPs/504s).
For Young Adults: We focus on how symptoms impact focus at work, project completion, organization, and daily adult responsibilities, providing clear next steps for career and lifestyle support.
