What Your Evaluation Includes:
A 45-minute comprehensive intake with a licensed nurse practitioner.
A 30-minute follow-up session to review evaluation forms.
A 30-minute executive functioning behavior training session
An official diagnosis (if criteria are met).
A personalized treatment plan.
One school accommodation (504/IEP) letter if needed
Followed by optional follow-up care for medication management, executive functioning coaching, and school accommodation (504/IEP) letters for $85 per 30-minute session.
Appointments are available via Telehealth anywhere in Arizona, or in-person at our Tempe office.
